National Home Care Agency is seeking a compassionate and dedicated Licensed Social Worker to join our team in Redford, MI. In this role, you’ll work closely with patients and families to navigate the emotional, social, and financial challenges that come with aging, chronic illness, and recovery, right in the comfort of their homes.
Key Responsibilities:
Conduct psychosocial assessments and develop individualized care plans
Provide emotional support and counseling to patients and families
Assist with advance care planning, crisis intervention, and resource coordination
Collaborate with clinical staff to ensure holistic patient care
Connect clients to community resources, benefits, and support services
Maintain accurate documentation and comply with all agency, state, and federal guidelines
Qualifications:
LMSW or LBSW license in the State of Michigan (Required)
Bachelor’s or Master’s degree in Social Work
1+ years of experience in home health, hospice, hospital, or similar setting (preferred)
Strong communication, advocacy, and problem-solving skills
Valid driver’s license and reliable transportation
